Yoshikawa was fired after a damning report into a collision between the Aegis-equipped DDG JDS Atago and a trawler, in which two fishermen were killed.
Apparently, various crew members including the one meant to be watching the radar were not at their posts- the BBC says that the captain and XO were both
asleep, but this is normal for a ship under way- unless the JMSDF has different practices. 80 other defence officials were fined or disciplined.

I was under the impression (from my time aboard USS Ranger in the late 1980s) that, barring planned drills, the Captain & XO had opposing watches, so one
would always be awake and available, even if he was in his cabin.

There have been a few scandals with the Defence bureaucracy recently - so the Admiral was given the boot.
